After you fill out the forms, take them to the clerk’s office. If you are submitting the forms with an Application for Fee Waiver, the clerk will tell you approximately when the decision will be made about the fees. Usually the clerk will provide a phone number for you to call. It could take up to two weeks for you to get an answer. Once you have the decision, you must go back to the clerk’s office to pick up the paperwork.
The clerk will sign the “Order For Hearing and Summons” part of the Motion for Modification and return the original forms to you. On the back/page 2 of the form, the clerk will also write in the date, time, and place of your hearing (your court date). The clerk will also sign some of the forms and return them to you.

You will need to make copies of the forms. Most courthouses have copy machines that you can use. You will need to make one copy for the other party and one copy for your own records. You will need to bring the original forms back to court.
